LAKEWOOD PARENTS!

“This was our daughter’s second year attending Camp Lakewood. She loved the counselors and all the activities that are packed into the day. I am impressed with the staff and counselors from different countries and feel that it is an important part of growing up to be exposed to people of different cultures. For you new camper parents, we thought you’d enjoy a quick informational video on what to expect when dropping off your child at camp.

FORMS

Below you will find all of the forms to complete the registration process for your child(ren) to attend camp. Some are optional depending upon your financial situation or if your camper wants to be involved with the CIT program, however all forms listed under PARENT PACKETS are required forms which must be completed prior to the start of camp. By sending in these forms ahead of time, you will save us and yourself a great deal of time, which you can spend on your very excited child the day their camp session begins.

REMEMBER! If registering prior to May 1, you may choose to pay the entire balance or a non-refundable deposit of $100 for each week requested to hold space. The deposit is applied to the total camp fee. If your deposit is paid by credit card, the balance will automatically be charged to your credit card on May 15. If the entire balance is not paid by May 15, the reservation may be cancelled and deposit forfeited. If you are registering after May 1, the entire balance is due upon registration. FOR THE PURPOSE OF COMPLETING THESE FILLABLE DOCUMENTS FROM YOUR DEVICE, PLEASE MAKE SURE TO SAVE THE BLANK DOCUMENT ON YOUR DEVICE FIRST, THEN RE-OPEN IN YOUR DEVICE TO FILL OUT, THEN SAVE AGAIN BEFORE SENDING/EMAILING BACK TO US. THIS WILL ENSURE THAT ALL FILLABLE FIELDS ARE VISUALIZED AND YOU HAVE A SAVED COPY OF WHAT YOU SUBMITTED TO US. YOU MAY ALSO JUST PRINT OFF THE FORM AND COMPLETE BY HAND.

Parent Handbook June 2020: After your child is registered for camp, you will be asked to read the Camp Lakewood Parent Handbook.

Parent’s Confidential Information & Parent Handbook Release Form: This form helps our staff to better understand the child in order to direct growth and development. This is shown only to relevant staff and is NOT shown to the child. There is also an area where parents must sign that they have read and understood the parent handbook.

Summer 2020 Waiver: This form for Summer 2020 in an addendum to our Parent Handbook and covers indemnity and our health screening rules and policies.

Health Form 2020:  You and your family doctor must complete this formIf your child has had a physical within the last 24 months, please attach a copy of the health exam. Per American Camping Association regulations and for the safety of all campers, the health form must be completed by the parent and a physician, along with signatures, and be in Camp Lakewood’s possession by your camper’s arrival, or the camper will not be able to attend camp.

Giving To Camp: If you feel it in your heart to help another child discover the joys of camp as much as your child will or has already experienced, please consider donating to our Annual Support Campaign, which helps children whose financial constraints prevent them from attending camp without this assistance.

Pick-Up Authorization: This form indicates whom you will allow to pick up your child from camp. Only the person/people listed on this form will be allowed to take the child home other than you. Anyone picking up a child MUST present a valid ID or driver’s license before the child will be allowed to leave the Camp Lakewood property. There will be no exceptions to this policy – your child’s safety comes first!

Special Smorgasbord Activity Form: This form gives you the option to add a one-time special smorgasbord activity for your child.

Trading Post-Warm Fuzzies: This form gives you the option to 1) place money in your child’s name to spend throughout his/her stay for snacks at our camp store and/or 2) purchase a special care package for your child from our camp store.

FINANCIAL AID   Click on form name to access (highlighted in orange if available; in black if pending). If you are in need of financial assistance, you must complete the Financial Assistance, Income Eligibility/Food Program forms (same form as in the “PARENT PACKET” section above).

Financial Assistance & Eligibility Form: To ensure that any child can attend camp regardless of financial ability, YMCA Camp Lakewood provides assistance through our Endowment Funds and the YMCA Annual Support Campaign. For more information on receiving financial assistance or if you would like to contribute to our financial assistance fund, please contact the Camp Lakewood office directly, at 573-438-2155 or via email at camplakewood@gwrymca.org.

Please scan and email all documents (Including proof of income and financial assistance form if you are requesting such assistance.) Applications are processed in the order they are received. Notification will be mailed to you as to what you qualify for within two weeks of receiving the application.

Open and save the pdf file onto your computer, and then type in the information and re-save before emailing. NOTE: You MUST save the form in your computer in order to be able to have a copy of the pdf form that you have already completed.


CLOTHING LISTS

Click on clothing list name to access. To update these lists on June 8, we added to all lists that each camper must bring multiple CDC recommended face coverings/masks and we removed the need for caving clothing from all camps except Rangers and LIT as we will not have off-site bus transportation this summer. Rangers and LITs we still be able to go caving as they hike to the caves rather than need a bus.
