For you new camper parents, we thought you’d enjoy a quick informational video on what to expect when dropping off your child at camp.
Below you will find all of the forms to complete the registration process for your child(ren) to attend camp. Some are optional depending upon your financial situation or if your camper wants to be involved with the CIT program, however all forms listed under PARENT PACKETS are required forms which must be completed prior to the start of camp. By sending in these forms ahead of time, you will save us and yourself a great deal of time, which you can spend on your very excited child the day their camp session begins.
REMEMBER! If registering prior to May 1st, you may choose to pay the entire balance or a non-refundable deposit of $100 for each week requested to hold space. The deposit is applied to the total camp fee. If your deposit is paid by credit card, the balance will automatically be charged to your credit card on May 1. If the entire balance is not paid by May 1, the reservation may be cancelled and deposit forfeited. If you are registering after May 1, the entire balance is due upon registration. PARENT HANDBOOK: After your child is registered for camp, you will be asked to read the Camp Lakewood Parent Handbook.
PARENTS’ CONFIDENTIAL INFORMATION & PARENT HANDBOOK RELEASE FORM: This form helps our staff to better understand the child in order to direct growth and development. This is shown only to relevant staff and is NOT shown to the child. There is also an area where parents must sign that they have read and understood the parent handbook.
HEALTH FORM: This form is for all pertinent information on the health of your child. This form must be completed within 12 months of attending camp.
FOOD PROGRAM ELIGIBILITY FORM: Even if you are not requesting financial assistance, your child may still qualify to receive free/discounted meals (a grant received to help offset cost of meals). We highly encourage you to complete the form if you meet the qualifying criteria listed in the form. This will help us to benefit from these offered subsidies. Thank you.
Note: If you are completing the Financial Assistance form (further down), you do not have to complete the Food Program Eligibility form, as it is already included in the Financial Assistance form.
PARENTS’ CONFIDENTIAL INFO & RELEASE FORM

PICK-UP AUTHORIZATION: This form indicates whom you will allow to pick up your child from camp. Only the person/people listed on this form will be allowed to take the child home other than you. Anyone picking up a child MUST present a valid ID or driver’s license before the child will be allowed to leave the Camp Lakewood property. There will be no exceptions to this policy – your child’s safety comes first!
TRADING POST-WARM FUZZIES: This form gives you the option to 1) place money in your child’s name to spend throughout his/her stay for snacks at our camp store and/or 2) purchase a special care package for your child from our camp store.
SPECIALIZED SMORGASBORD FORM: This form gives you the option to add a one-time special smorgasbord activity for your child.

If you are in need of financial assistance, you must complete the Financial Assistance, Income Eligibility/Food Program forms.
To ensure that any child can attend camp regardless of financial ability, YMCA Camp Lakewood provides assistance through our Endowment Funds and the YMCA Annual Support Campaign. FINANCIAL ASSISTANCE, FOOD PROGRAM & INCOME ELIGIBILITY FORMS
Camp Lakewood offers a 20% discount off of the main camp and east camp program fees (only). Of course, one of the founding principles of the YMCA is that no one is turned away due to inability to pay. If additional assistance is needed, we welcome financial assistance applications and will gladly work with military families to make sure their children get the opportunity for a great summer camp experience!
Simply complete the registration form as usual (either online or hard copy), and discount the main camp or east camp program pricing (only) by 20% and enter that amount in the PAYMENT INFORMATION section under “Program Price”). You will be required, upon check-in, to provide the staff with your military ID confirming your active duty status. Please scan and email all documents (Including proof of income and financial assistance form if you are requesting such assistance.) Applications are processed in the order they are received. Notification will be mailed to you as to what you qualify for within two weeks of receiving the application.

Open and save the pdf file onto your computer, and then type in the information and re-save before emailing.
NOTE: You MUST save the form in your computer in order to be able to have a copy of the pdf form that you have already completed.
